Terms of Service | Ind Reports

1. Agreement

These Terms of Service (“Terms”) govern your access to and use of the Ind Reports website and report generator (the “Service”) at https://indreports.com. By using the Service, you agree to these Terms. If you do not agree, do not use the Service.

2. What Ind Reports is — and is not

Ind Reports provides software tools that help authorized clinical staff format and print medical-style reports on your own facility letterhead.

Ind Reports is not a medical laboratory, diagnostic provider, employer, immigration authority, or healthcare provider. We do not perform tests, verify results, certify fitness, or make clinical decisions. Output from the Service is only as accurate and legitimate as the information you enter and the authority your organization has to issue it.

3. Who may use the Service

The Service is intended for use by authorized personnel of licensed or otherwise legally permitted clinics, laboratories, hospitals, and similar healthcare organizations, acting in a professional capacity.

You represent that you have the authority to use the Service on behalf of your organization and that your use complies with all laws, regulations, and professional standards that apply to you.

4. Prohibited conduct — no false or fraudulent reports

You must not use the Service to create, print, distribute, or present reports that are false, fabricated, misleading, or issued without proper testing, authorization, or clinical basis. This includes, without limitation:

Entering results you know to be untrue or unverified;
Impersonating another facility, laboratory, or practitioner;
Using the Service to obtain employment, travel, insurance, immigration, or other benefits through misrepresentation;
Altering or backdating reports to conceal the true origin or timing of testing;
Using templates in a way that suggests Ind Reports (rather than your facility) performed or certified the testing.

5. Your responsibility and liability

You are solely responsible for all patient information, test results, clinical judgments, signatures, stamps, and the final printed or digital documents you produce. You are liable for any harm, loss, regulatory action, or legal claim arising from your use of the Service, including the creation or distribution of false, fraudulent, or unauthorized reports.

Ind Reports does not review, approve, or validate your entries. Reference-range highlighting and formatting features are aids only; they do not constitute clinical validation.

6. Indemnification

You agree to defend, indemnify, and hold harmless Ind Reports, its operators, and affiliates from any claims, damages, fines, penalties, costs, and expenses (including reasonable legal fees) arising from your use of the Service, your reports, your violation of these Terms, or your violation of any law or third-party rights.

7. Patient and report data

Report and patient data entered in the generator are processed in your browser session for preview and printing. Ind Reports does not intentionally store completed report content on our servers as part of normal report generation. You are responsible for securing devices, printouts, and PDFs you create, and for compliance with privacy laws applicable to you (for example HIPAA, GDPR, or local health-privacy rules).

8. Disclaimers

THE SERVICE IS PROVIDED “AS IS” AND “AS AVAILABLE.” TO THE MAXIMUM EXTENT PERMITTED BY LAW, IND REPORTS DISCLAIMS ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE SERVICE WILL BE ERROR-FREE, UNINTERRUPTED, OR SUITABLE FOR ANY PARTICULAR REGULATORY OR CLINICAL PURPOSE.

9. Limitation of liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, IND REPORTS AND ITS OPERATORS WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR FOR ANY LOSS OF PROFITS, DATA, GOODWILL, OR REGULATORY SANCTIONS, ARISING FROM YOUR USE OF THE SERVICE. OUR TOTAL LIABILITY FOR ANY CLAIM RELATING TO THE SERVICE WILL NOT EXCEED THE GREATER OF (A) THE AMOUNT YOU PAID US FOR THE SERVICE IN THE TWELVE MONTHS BEFORE THE CLAIM, OR (B) ONE HUNDRED U.S. DOLLARS (USD $100).

10. Suspension and termination

We may suspend or restrict access to the Service at any time if we believe you have violated these Terms or if necessary to protect the public, comply with law, or protect our systems. Provisions that by their nature should survive (including Sections 4–9) will survive termination.

11. Changes

We may update these Terms from time to time. The “Last updated” date above will change when we do. Continued use after changes constitutes acceptance of the revised Terms.
